This year the members of the Intercollegiate Academic Clinical Coordinators Council (IACCC) will award two physical therapy students scholarships in the amount of $2500 each and two physical therapy assistant student scholarships in the amount of $1500 each. These scholarships are based on merit and will be awarded to students who have completed their first full-time (a minimum of five weeks) affiliation as part of their physical therapy education.

The application must include the following materials (**Please submit them in the order listed):

a) Completed application form (including professional, school and volunteer work limited to the past    three years and not required by the PT or PTA program)

b) Typed essay on a subject or ethical scenario selected yearly by IACCC

c) From the Clinical Performance Instrument (CPI)

A copy of the CI’s portion of the CPI, including either visual analog scales or categorica; ordered scales, summative (final) and formative (midterm) written comments, from your first fulltime affiliation.

d) Scholarship Recommendation Form completed by the Clinical Instructor of the first affiliation

e) Verification of APTA membership required at the time of the first affiliation and at the time of the         scholarship award

f) Verification of Grade Point Average, number of completed units, and number of remaining units for completion of program
